Transparent Physical Therapy Pricing

Horizon is a cash-based, out-of-network physical therapy practice. We do not bill insurance directly. You will pay for your care at the time of service and can request a superbill to submit to your insurance company for potential out-of-network reimbursement.

Your Investment Includes

  • 60-minute, one-on-one sessions — Your appointment is dedicated entirely to you, with no double-booking or rushed visits.

  • Personalized care — Every treatment plan is tailored to your symptoms, goals, and lifestyle and not dictate by an insurance company.

  • Transparent pricing — You’ll know exactly what your session costs before you book, with no surprise bills.

  • Insurance reimbursement support — Horizon is an out-of-network practice. You’ll receive a superbill after your visit that you can submit to your insurance for potential out-of-network reimbursement.

  • Personalized home program — You’ll have access to video demonstrations and clear guidance so you know exactly what to work on between sessions.

  • HSA/FSA eligible — Services may be eligible for payment with HSA or FSA funds.

Why Cash-Based Care?

Traditional insurance-based PT often means shorter appointments, larger patient caseloads, and limited treatment options based on what insurance will cover. At Horizon, you’re paying for dedicated time, individualized care, and the flexibility to focus on what you actually need.

PRICING

Initial Evaluation | $195
60 minute appointment including a comprehensive evaluation and personalized plan for care.

Follow-Up Appointment | $195
50–60 minute appointments focused on your treatment, progress, and goals.

Cancellation & Rescheduling Policy:

• Cancellations or reschedule requests made 24 or more hours before your appointment may be made at no charge.

• Appointments canceled with less than 24 hours notice will incur a $50 late cancellation fee.

• No-show appointments will automatically be charged the full cost of the scheduled session.

Do I need a referral for therapy?

A referral is not required to get started.

Pennsylvania allows patients to seek physical therapy directly without a physician referral. As a Pennsylvania Direct Access-certified physical therapist, Dr. Nina can evaluate and treat you without a referral for up to 30 calendar days from your first treatment.

If continued physical therapy is appropriate beyond the 30-day direct-access period, we’ll let you know what type of referral is needed and help guide you through the process.
